Fellowship Program
During your 12 months at Derriford Cardiothoracic Unit you will gain exposure to a broad range of cardiothoracic procedures. A selection of some of the cases we currently manage are detailed below:
Cardiac (100 cases) [7 months]
- CABG, AVR, MVR, TVR
- Mini-mitral repair
- Type A Aortic dissection repair including DHCA
- Aortic reconstruction including frozen elephant trunk (FET)
- TEVAR (in interventional radiology suite)
- MAZE procedure and left atrial appendage clip
Thoracics (25 cases) [6 weeks]
- Open and VATS lobectomy, wedge resection and segmentectomy
- Robotically assisted thoracoscopic surgery for thymectomy and lung resection
- Shared airway rigid and flexible bronchoscopy
- Rib fixation
- Lung isolation techniques, fibreoptic bronchoscopy, nasendoscopy etc…
Vascular (25 cases) [4 weeks]
- Open AAA repair including aortobifemoral
- Axillobifemoral bypass grafting
- EVAR in interventional radiology suite
- Awake carotid endarterectomy
Echocardiography [2 weeks incorporated into 12 month program]
- Fortnightly TOE and TTE teaching provided by our faculty
- Ability to discuss difficult TOE and TTE scans at weekly echo meeting
Interventional cardiology [2 weeks]
- Transaortic valve implantation (TAVI) via transfemoral, apical and subclavian routes
- Transcatheter “edge to edge” repair of mitral valve (PASCAL)
- Permanent pacemaker insertion
- Complex lead extraction
- Electrophysiology procedures including AF and atrial flutter ablations
Intensive Care [4 weeks]
- Post operative care for all cardiac surgical cases listed above
- Advanced haemodynamic monitoring including swan-ganz catheter and PICCO
- Bedside TOE for haemodynamic assessment on ICU
- Interventional procedures (central venous access, arterial access, bronchoscopy etc…)
- Percutaenous tracheostomy
Perfusion [2 weeks integrated into 12 month rotation]
- Allocated time on the rota to be on a list with perfusionist
- Dedicated teaching sessions as part of our teaching rota on perfusion topics
- Teaching on VA and VV ECMO
