Antibiotic Prophylaxis for Cardiac Surgery

Cardiac Surgical Patients

Primary procedure

Teicoplanin (10mg/Kg, with a second dose to be given after 12 hours) plus a single dose of Gentamicin (3mg/Kg)*.

Repeat procedure (re-sternotomy)

Within 28 days of primary operation – Meropenem 1g TDS for 48hrs plus Teicoplanin (10mg/Kg with a second dose to be given after 12 hours) **

*If eGFR <20ml/min then discuss with microbiology

** If already received a dose of teicoplanin within the preceding 12 hours then ensure subsequent doses are delayed accordingly. (Teicoplanin not to be dosed more frequently than BD)
